Understanding the Impact of COVID-19 on Aged Care Facilities

Chapter 1: The Reality of Aged Care During the Pandemic
This article is inspired by a comprehensive study conducted by my colleague, Prof. Andrew Levin, and his team, published in the European Journal of Epidemiology. Their work investigates the effects of the COVID-19 pandemic on aged care settings.
One crucial discovery throughout this crisis has been the age-related vulnerability to COVID-19. A study we conducted back in 2020 demonstrated that the likelihood of death from COVID-19 escalates significantly with age. Specifically, a 60-year-old faces about ten times the risk of dying compared to a 40-year-old, yet is still considerably less likely to die than an 80-year-old.
Despite these findings, a pressing question remained as the pandemic progressed: What was the situation in aged care facilities? While elderly individuals were indeed at a higher risk of mortality, our initial study did not focus on the unique dynamics of aged care. Some have argued that the pandemic's threat is not as pronounced for younger, healthier individuals outside of nursing homes.
Would the pandemic's trajectory have been different without the presence of infections in aged care facilities? The response to this question is multifaceted. Reducing infection rates in these facilities likely would have significantly lowered the overall death toll from COVID-19. However, there is little evidence to suggest that residing in a nursing home inherently increases one's risk of death from the virus. Although mortality rates in aged care settings are notably higher than in the general community, this discrepancy is primarily due to the fact that residents are generally older and more susceptible to the virus.
Section 1.1: Defining Aged Care
The concept of aged care may seem straightforward, yet it is surprisingly intricate.
Essentially, aged care refers to specialized facilities known as Long-Term Care Facilities, where individuals receive extensive assistance due to serious health issues. These establishments often provide round-the-clock nursing care and are subject to strict regulations.
However, the situation varies greatly from one country to another. For instance, in Spain, 30% of older adults live with their children, whereas in Sweden, this figure is a mere 1.5%. This diversity means that nursing home populations can differ dramatically, impacting health outcomes and living conditions significantly.
Section 1.2: COVID-19's Impact on Aged Care Facilities
During the pandemic, aged care facilities did not seem to offer much protection against COVID-19. In fact, early evidence indicates that residing in such facilities heightened the likelihood of contracting the virus.
The ease of transmission in these environments contrasts sharply with the ability of elderly individuals living independently in high-income nations to isolate themselves. Furthermore, a critical inquiry arises: Did individuals in aged care face a higher fatality rate if they contracted the virus?
To explore this, we can analyze infection fatality rates (IFR) through serology studies, which examine antibody presence prior to vaccination. Recent reviews suggest that aged care residents exhibited a death rate roughly double that of individuals living in the community. However, this statistic can be misleading.
Individuals in aged care tend to be older than their community counterparts, and when we adjust for age, the difference in death rates largely vanishes. Evidence from countries like Spain, Sweden, the UK, and the USA reveals that aged care residents are typically about a decade older than those aged 70 and above living outside these facilities.
Taking age into account suggests that residing in a nursing home does not significantly increase the risk of dying from COVID-19 once infected. The higher mortality rates among aged care populations primarily stem from their advanced age, rather than any inherent dangers associated with aged care itself.
Chapter 2: Lessons Learned and Future Considerations

The key takeaway is that significant improvements were necessary in our management of nursing homes, especially in the initial stages of the pandemic. Since February 2020, we’ve understood that elderly individuals are at the highest risk for severe outcomes from COVID-19. The fact that living in these facilities appeared to increase the risk of contracting the virus is a serious indictment of our pandemic response.
However, it should also be noted that even if we had effectively addressed the issues in nursing homes, the pandemic would still have posed a grave threat to the elderly population. The evidence indicates that COVID-19 was a highly lethal disease, and the challenges faced by nursing homes were part of a broader public health crisis.
